STHS impact concussion test and Form D-Physical

If you are interested in playing a sport in the 2025-2026 season, please complete the required yearly online Concussion Test and NIAA Form D-Physical Form. Links below!

Concussion Test:
Follow the Instructions Below. Use Team Code H9W08HT1

Steps:
1) Using your smartphone - Download the HitCheck App. (Apple App Store or Google Play)
2) Once Downloaded, Click create an account. (account must be under Athletes name. not parent)
3) Create Account (Fill in all required Information). Use Team Code H9W08HT1. Click Submit.
4) Take the Baseline Test.
