Sports Physicals for High School Athletes! If you are interested in playing a sport in the 2025-2026 season, you will need to complete the required Physical Form every school year.
Go to Aktivate.com (Registermyathlete.com). ** South Tahoe High School is under the State of Nevada. Not California **
You can Download a copy of the Physical Form from Aktivate.com (Form D Physical 2026) or come by the front office at STHS and pick one up from the Athletic Secretary.

Follow the Instructions Below. Use Team Code H9W08HT1
Steps:
1) Using your smartphone - Download the HitCheck App. (Apple App Store or Google Play)
2) Once Downloaded, Click create an account. (account must be under Athletes name and School Email. not parent)
3) Create Account (Fill in all required Information). Use Team Code H9W08HT1. Click Submit.
4) Take the Baseline Test.
